Use this form to request access, correction, deletion, portability, opt-out, restriction, third-party information, or appeal a prior decision.
Do not submit passwords, payment-card numbers, government IDs, medical records, or other sensitive documents. We will contact you if identity or authority verification is needed.
You may instead email [email protected] with “Privacy Request” or “Privacy Appeal” in the subject.
